SUBJECT 1: Breast Milk Depots in Missouri
SUBJECT 2: Revised WIC Notification of Ineligibility or Termination Form (WIC 19)

  1. Breast Milk Depots in Missouri:  Missouri now has two milk depots that provide breast milk to the Mothers’ Milk Bank located in Denver, Colorado.  The milk depots are located at St. Luke’s Hospital in Kansas City and Fitzgibbon Hospital in Marshall, Missouri.  Milk provided to the Mothers’ Milk Bank provides screened donor milk to premature and ill babies throughout the country.  In order to be a candidate for donation, moms need to have a baby less than 1 year of age, be a non-smoker, taking no medications or herbs and willing to donate a minimum of 150 ounces.  Please have possible donor candidates contact St. Luke’s Hospital at 816-932-4888 and Fitzgibbon Hospital at 660-831-3710 for additional information.  For more information on the Mothers’ Milk Bank please go to www.milkbankcolorado.com.
  2. Revised WIC Notification of Ineligibility or Termination Form (WIC 19) in 16 Languages:  The WIC 19 form (01-12 version) is available in the following languages: Arabic, Bosnian, Burmese, Chinese, English, Farsi, French, Hmong, Korea, Romanian, Russian, Somali, Spanish, Ukrainian, Urdu, and Vietnamese.  Please utilize them as needed.  If you have any questions, please feel free to contact Takako Tagami at Takako.Tagami@health.mo.gov.
